Classified under table 1 by Italian Misuse of Drugs Act (DPR 309/90)

Synthetic cathinones, such as mephedrone, have a central action similar to amphetamines.

Mephedrone is a white or yellowish powder, often in crystals to grind before snorting.

Habitually mephedrone is sniffed, or swallowed (“bomb”) like a pill, wrapped in a cigarette map. It can be injected or taken orally, after solution in water.

Mephedrone is sold in 1 gram sachets, corresponding to the snorted quantity per dose.

The effect occurs after a few minutes when snorted and lasts for 2-3 hours. To maintain the desired effect, they generally take mephedrone several times during the same session. Mephedrone very often is laced with other substances. it is difficult to determine and individuate toxic effects due to mephedrone from those of lacing substances, generally other amphetamine-like stimulants.

DESIRED EFFECTS
Stimulant effects, as by amphetamines, cocaine and ecstasy. Euphoria. Increased concentration and ability to move, to speak and to enjoy music.
SEX AND MEPHEDRONE
Mephedrone enhances erotic fantasies, desire and sexual performances. Disinhibition of sexual behavior, with longer sexual session for men and greater orgasm’s intensity.

Viagra may be requested to obtain satisfactory erection and orgasm.

Mephedrone is often associated with the GHB/GBL to counteract their sedative action.

Mephedrone use, alone or in association to other chems, reduces sexual inhibitions, with increased risk to contract sexually transmitted infections (such as Hiv and hepatitis C) and can result in sexual actions you would not desire as sober.
TOXIC EFFECTS AND HARMS
Harms due to mephedrone: agitation, fast heartbeat, increased body temperature, palpitations, vomiting, increased blood pressure, chest pain, severe headache and convulsions. Mephedrone very often is laced with other substances. it is difficult to determine and separate toxic effects due to mephedrone from those other substances, generally other amphetamine-like stimulants. After vein injection, very frequently mephedrone unleashes psychiatric and neurological effects: paranoia, suicidal fantasies, aggressive and violent behavior, tremors and rigidity as in Parkinson’s patients.

Teeth damage due to involuntary grinding. Appetite reduction. Sore nasal passages and nostrils ulcers.

Nausea and vomiting.

Insomnia and nightmares. Alteration of the ability to focus images and to think precisely. Memory problems. Hallucinations, anxiety, headache.
DEPENDENCY AND WITHDRAWAL
Serious addiction is not described, but mephedrone causes craving, with fast increase of the quantity needed to achieve the desired effect (tollerance).

Severe abstinence is not described, but habitual administration causes agitation, general discomfort and continuous movement.
INTERACTION WITH PRESCRIPTION AND RECREATIONAL DRUGS
Association with alcohol enhances mephedrone’s effects and can lead to severe cardiac arrhythmias.

Association with other stimulants increases toxicity.

Some oral antifungal (ketoconazole, itraconazole, fluconazole), Macrolide antibiotics (erythromycin,clarithromycin, troleandromycin) and cimetidine, are potent inhibitors of mephedrone elimination, like some natural components present in grapefruit juice (narigenin, quercetin).

Prescription drugs increasing mephedrone toxicity: amiodarone, citalopram, codeine, fluoxetine, haloperidol, methadone, paroxetine, valproic acid.

Antiretroviral cobicistat slows down the metabolism and increases mephedrone toxicity.

Habitual users of mephedrone do not follow properly antiretroviral therapy.

EDA (Agents of erectile dysfunction) sildenafil, tadalafil and vardenafil, also called Viagra, taken together with mephedrone are eliminated more slowly. Cobicistat and ritonavir lead to a longer EDA duration. Toxic effects on heart and circulation are enhanced by the combined action of these substances.
TIPS FOR SAFER USE OF MEPHEDRONE
